Anonymous inquiry for Covid-19 testing

Dear friends,

To be able to properly organise the COVID-19 testing, we ask all of you who need to test for COVID-19 to fill this ANONYMOUS form so we can organise the testing in a fast and efficient manner.

We remind you that to be able to enter our event(s) you need to have a valid EU Covid-19 passport. Read more about COVID-19 regulation for 2021 Seasunsalsa events.

IMPORTANT

• YOU DO NOT NEED TO FILL THIS FORM if you have been vaccinated in the last 270 days or had a corona virus in the last 6 months and you have an EU COVID-19 passport
• Self-bought tests are not enough. We can only accept tests done by official medical staff/lab which can issue a QR code (EU Covid-19 passport) that we can check and read with the official CovidGO application (available for Android OS and iOS).
